Forms

The following forms are used in NFC's eAuthentication registration process. All of these forms are available in Adobe Portable Document Format (PDF). To view and print PDF files, you must have Adobe Acrobat Reader v3.0 or above, or the equivalent browser plug-in, installed on your computer. • Certificate Action Request (CAR) Form, Form AD-1148. The first step in the registration process is to complete the CAR form. The form requires Identification Verification of the applicant. The identity verification can be completed by either the Local Registration Authority (LRA) or a Notary Public. Once identity verification is complete, the form must be forwarded to NFC for retention. To download a copy of the CAR, click here.
  
  

Subscriber Agreement, Form AD-1149. Each Subscriber must complete and sign a Subscriber Agreement in the presence of the LRA before activating his/her certificate. The completed Subscriber Agreement is provided to the LRA with a copy(ies) of picture ID's. The LRA mails the completed Subscriber Agreement with a copy(ies) of picture ID's to NFC for retention. • LRA Agreement, Form AD-1150. Anyone who is going to act as an LRA for an agency must complete and sign an LRA Agreement. The LRA must also complete a CAR and a Subscriber Agreement since they will be using a certificate in the LRA process. To download a copy of the LRA Agreement, click here.
